This is an old Tiffany apple key ring. By twisting the catch ball like a screw, the tip comes off, allowing you to bundle keys or customize it by attaching it to bags or rider's jackets. For reference, I've included an example of it attached to the zipper of a vintage rider's jacket. The apple motif makes it suitable for both men and women. It's made of solid Silver 925 and the hallmarks are clearly visible. Being vintage, there are some minor scratches, scuffs, and tarnish from use, but it adds to the overall cool aesthetic!
