The model number is unknown, but it appears to be equivalent to the CL-005 sold on Amazon, etc. (There is no model number indicated anywhere.) The side of the lens is marked with "4K HD 16mm Wide Angle Lens No Distortion." The part that screws onto the smartphone has a 17mm screw thread. The paint is slightly chipped due to friction in that area. I'm an amateur, but I don't see any scratches on the lens. There's also something that looks like a filter, so I'll include it as a set.
