You can put your hand in the lace part to firmly hold the baby's head, preventing the pillow from slipping or falling. I've included a photo with a pink sticky note (not for sale) inserted as an example. The area where I wrote my name on the tag has been marked out with a marker. There are no noticeable scratches or stains, and it has been hand-washed, but please consider purchasing only if you understand that it is used.
