This is an HD remastered DVD released in 2011. Includes a shrunk-down version of the pamphlet as a first-press limited bonus. It was originally sealed inside the DVD case, so it was creased from the beginning (see image 5). Both the disc and the jacket appear to be in good condition with no noticeable scratches, but please refrain from purchasing if you are overly concerned about the condition, as this is a used item.
