There is some deterioration of the rubber on the black screw receivers and tape residue on the back from being attached to an effects board, but it's in relatively good condition considering its age. I have confirmed that it produces sound with an adapter, but I have not confirmed sound output using a 9V battery. It works without problems, and there is some slight crackling, but please understand that this is common with older pedals.
