I believe I purchased this several years ago, probably at either the Beams Marunouchi store or the Mitsukoshi Nihombashi Main Store. I haven't worn it much recently, and it's been stored after being dry cleaned. There was some yellowing around the collar, so I had it professionally stain-cleaned. Regarding the size, I'm not very good at measuring, so please refer to the size in the images.
