Includes the adapter and all other original accessories; nothing is missing from the original purchase. I assembled it on a pedalboard, but the board became unexpectedly heavy, so I disassembled it. I've been storing it, waiting for a chance to use it in a practical setting, but never actually did. I'm selling off some gear, so I'd like to offer it to someone who needs it. Since it hasn't been used in a practical setting, I think it's in near-new condition. I did apply Velcro once, but I removed it quickly, so there are no major marks left.
