I believe this is a supplement from the December 1999 issue of Gekkan Kayokyoku (Monthly Kayokyoku). I haven't measured it precisely, but it looks to be around A2 size. It has been stored in a dark place, so it's relatively clean, but it's been over 25 years, so please consider purchasing only if you understand this.
