This is believed to be an included bonus from the Yu-Gi-Oh! Morinaga Choco Snack, which was released in the late 1990s. It is a rubber mini-figure of Red-Eyes Black Dragon. It's a small item, about 3cm tall. The items in the photos are all that are included, just the figure itself. I believe there is no particular damage, but as it is an old item, there is some age-related deterioration, minor scuffs, scratches, and dirt.
