This is a single infantry piece (歩兵) with a Nishiki-ki script by Seizan Kanai. It is a moriage koma (raised lettering), but it has been used quite a bit, and both the front and back are in a hori-ume (carved and filled) state. (It seems Seizan Kanai only made moriage koma.)
