Comme des Garçons Homme Plus AD1999. I believe this item is from the 99AW collection, commonly known as the "Hermès" era. It's a thin, gingham check jacket, and the peak lapels on a single-breasted jacket are a very Comme des Garçons touch, which I personally like.
