The model number is SL-012BKS. I purchased it several years ago, but I only used it a few times and then stored it. Since it has been used so few times, I think it's in relatively good condition. There are marks on the back of the brush handle that won't come off even if wiped, and there are some minor scratches. Also, the warning paper attached to the cord is creased. I have confirmed that it works and wiped it with a disinfectant sheet, but please understand that it is a used item. *There is no box or instruction manual.*
